36. Establishing Relationships with Community Resources for Community-Clinical Linkages

This session will provide participants with insights on how to identify community resources; establish formal relationships with community resources; and maximize ease of communicating patient data back and forth. This session will also address formal data sharing agreements between a HIPAA-covered entity (clinic) and a non-covered entity (community organization) such as via an Memorandum of Agreement or Understanding (MOA/MOU) and a Business Associates Agreement (BAA), and help awardees consider the need for referral and tracking in the absence of options in the clinic’s current EHR to receive communication from a community organization.

  1. Describe methods for identifying community resources to which the priority population might be referred.
  2. Identify software on the market that might meet the need for referral and tracking in the absence of options in the clinic’s current EHR to receive communication from a community organization.
  3. Describe the benefits of formal data sharing agreements between a HIPAA-covered entity (clinic) and a non-covered entity (community organization).
