36. Establishing Relationships with Community Resources for Community-Clinical Linkages

This session will provide participants with insights on how to identify community resources; establish formal relationships with community resources; and maximize ease of communicating patient data back and forth. This session will also address formal data sharing agreements between a HIPAA-covered entity (clinic) and a non-covered entity (community organization) such as via an Memorandum of Agreement or Understanding (MOA/MOU) and a Business Associates Agreement (BAA), and help awardees consider the need for referral and tracking in the absence of options in the clinic’s current EHR to receive communication from a community organization.

- Describe methods for identifying community resources to which the priority population might be referred.
- Identify software on the market that might meet the need for referral and tracking in the absence of options in the clinic’s current EHR to receive communication from a community organization.
- Describe the benefits of formal data sharing agreements between a HIPAA-covered entity (clinic) and a non-covered entity (community organization).
